Physical Therapy Self Pay & Diagnosis Care Packages 

Self pay for an Initial Evaluation $150

Self pay for regular visits  $95

Care Packages-
Least evasive diagnosis care package, based on a one month prognosis $950

Moderate diagnosis care package, more involved conditions with a 6 week prognosis $1,390

Complex cases and post surgical events, 3 month prognosis $2,690

*The appropriate package is to be determined by the evaluating therapist, and deciding factors are directly related to the patient’s report and therapist’s findings.  The number of visits is generally 2-3 times per week. Care packages are to be paid in full at the initial visit, or pay per visit fees will apply. 

In addition, we accept most major medical plans. If you don’t see your insurance listed under our accepted plans, please call us and we’ll make arrangements to add your insurance carrier
